Condo Act Forms for Owners
As of January 1, 2020, the Ontario Ministry of Government and Consumer Services has delegated the hosting and maintenance of various Condominium Act Forms to the Condominium Authority of Ontario (CAO). These include forms such as:
- Request for Records
- Submission to Include Material in the Notice of Meeting of Owners
- Agreement to Receive Notices Electronically
- And many others
Please note that the “Request for Records” form prescribed by the Condo Act is a mandatory form and must be used as per Reg. 48/01: s. 13.3 (3). Once downloaded from the CAO and completed in full, the “Request for Records” form should be sent to:
